Welcome to Chequamegon Clinic

The new St. Luke's Chequamegon Clinic offers patients lakefront views throughout the clinic for a healing environment and enhanced patient experience. We are located between Culver's and Ashland Honda Toyota.

We have eight family medicine physicians and one nurse practitioner with room for more providers as the clinic grows. St. Luke's outreach specialists visit the clinic on a regular basis including cardiology, endocrinology, general surgery, nephrology, neurology, OB-GYN, orthopedics and sports medicine, pulmonary medicine, urology, and vascular surgery. New services at the clinic will include 3D mammography, physical therapy, and ultrasound to meet the needs of our patients.

At Chequamegon Clinic, our focus is you. Our staff is experienced, compassionate and dedicated to promoting the health of individuals and families in Ashland and the surrounding area, supported by the expert resources of St. Luke's.

After Hours/Emergency Care

There is always a St. Luke's physician on call after clinic hours. Please call the clinic number. If you need immediate care for a condition that is not life-threatening, go to the nearest Urgent Care.

If you or a family member is experiencing a life-threatening injury or illness, call 911.


Prescription Refills

If you need a prescription refilled, contact your pharmacy. They will work with your St. Luke's doctor to coordinate your refill. If your medication was prescribed by another doctor, you'll need to contact that doctor directly if your pharmacy is unable to fulfill your request.


Lab Procedures, Nurse Visits and Tests

Please call in advance to come in for lab procedures or tests (such as allergy shots, blood pressure checks or lab work) that don't require a visit with your doctor. Calling ahead gives us time to prepare your chart, which will reduce your time in the waiting room.


Referrals

If you need to see a specialist, we will help you coordinate your care. Please let us know if your health plan requires a referral or prior authorization. If you have questions, please call the clinic number and ask to speak to someone about referrals.


Billing and Insurance

St. Luke's medical clinics accept Medicare, Medical Assistance and most commercial insurance and health plans. If you have questions about insurance coverage at our clinic, please call your insurance or health plan.

Co-pays or minimum cash payments are collected at the time of your office visit. We will bill your primary and secondary insurance claims for you. After payment is received from your insurance provider, you will be billed for deductibles, additional co-pays and services not covered by your insurance. A finance charge of 1 percent per month may be applied to patient balances that are more than 90 days past due.

For elective procedures not covered by insurance, please talk to the business office about payment arrangements before you have the procedure.

St. Luke's accepts Visa, MasterCard and Discover. If you need help with payment arrangements, please talk to a patient account representative at your clinic.
