Short-Term Rehab Program
Supporting return to daily life
Lake View Hospital in Two Harbors, part of the St. Luke’s system,
offers a short-term rehab program. Also known as a swing bed program,
it offers a place for people to stay after hospitalization to receive
personalized rehabilitation care.
Short-term rehab is often appropriate for people who require the following:
- Post-surgery rehabilitation
- Specialized wound care
- Stroke rehabilitation
- IV therapy
- Cardiac rehabilitation
- Pain management
Whether a patient was hospitalized at Lake View, St. Luke’s or any
other facility, the patient can be transferred to Lake View. The skilled
nursing staff and therapists will work with patients to help them regain
strength, function and independence.
Services
The Lake View team develops a customized plan for each patient. Typically,
the plan includes twice-daily sessions with a physical therapist, plus
regular sessions with occupational and speech therapists. Patients may
stay for as little as a week or for up to 100 days. The average stay is
two to three weeks.
Rehab Apartment
Lake View offers a unique inpatient apartment that allows caregivers the
opportunity to assess a patient’s ability to safely move around
in a home environment. This feature helps inform discharge planning and
gives patients, families and caregivers confidence in the plan.
Medicare Requirements
Medicare Part A will generally pay for short-term rehab (referred to as
“Swing Bed”) if a patient has been hospitalized for three
consecutive days and the patient’s doctor determines that daily
skilled care is needed. If these criteria are met, Medicare will pay all
covered charges for 20 days. After that, Medicare will pay covered charges
except for the skilled nursing coinsurance. Other insurances will also
