Home Telemonitoring

Helping patients stay healthy from the comfort of home

St. Luke's offers home telemonitoring technology for home care and hospice patients. This technology allows St. Luke's medical staff to monitor patients who no longer require hospitalization and could benefit from close observation after discharge.

A monitoring unit is placed in the home to measure blood pressure, heart rate, oxygen levels and weight each day. A voice prompt provides instruction at the appropriate time each day and results are sent via the telephone line to St. Luke's. A nurse reviews and evaluates the information to provide follow-up as needed.

If you qualify for skilled home health care, this service is included as a benefit. St. Luke's can help you determine if you meet eligibility for this monitoring. No computer skills are needed, and a nurse will make sure you understand how to use the equipment.

Home telemonitoring is available for a daily fee for patients no longer eligible for Medicare-covered home health service. Home telemonitoring is also available to patients who have never had skilled home health services on a private-pay basis.

Home telemonitoring technology is funded, in part, by a grant from St. Luke's Foundation.
