St. Luke's Home Care offers the assistance, knowledge and support to keep patients independent
in the comfort of their homes. St. Luke's provides a full range of
services for patients after a hospital stay or surgical procedure or for
those who are unable to leave their homes for medical care and appointments.
Services are available for patients who live within 30 miles of
St. Luke's Hospital in Duluth,
Lake View Hospital in Two Harbors and Community Memorial Hospital in Cloquet.
St. Luke's Home Care is accredited by the Joint Commission and is Medicare-certified
and state licensed in Minnesota and Wisconsin.
Extended Care Services
St. Luke's Home Care offers extended care services for patients who
would like additional help beyond the services prescribed by a physician.
Extended Care Services include bathing, grooming, meal preparation, errands
or grocery shopping. Services are usually on a private-pay basis. If you
have questions about coverage for your care, please contact us for assistance.
Referral for Home Care
Home Care referrals may be made by a health care provider, family member,
friend or self-referral. Once a referral is made, our intake staff will
contact the health care provider to discuss the services needed and obtain
physician orders to initiate care.
Insurance Coverage
Services provided by St. Luke's Home Care are often covered through
Medicare, Medicaid, your health care plan or private insurance. If you
