St. Luke's Community Care Team

St. Luke’s Community Care Team visits several area assisted living facilities and nursing homes to care for residents. Our dedicated team includes physicians, nurse practitioners, and a care coordinator who visit each facility on a regular basis to care for patients. By having the team travel to patients, care can be provided as conveniently and comfortably as possible.

Current St. Luke’s patients will automatically be enrolled in the program, and anyone who wishes to enroll themselves or a loved one may contact the St. Luke’s Community Care Team.
